What are Self-Management Workshops?

These workshops empower participants and caregivers to be an active partner in their healthcare. Participants engage with certified leaders and learn techniques to deal with stress, pain, and fatigue, strategies to eat well and exercise safely, and more. Each workshop is 6 weeks and are available either online or in-person.

Chronic Disease Self-Management

Open to any age with any condition, including caretakers. Learn techniques to work with doctors, manage symptoms, set realistic goals, and gain strategies to eat well and safely participate in exercise.

Diabetes Self-Management

Our Diabetes Self-Management Education (DSME) Program is accredited by the Association of Diabetes Care and Education Specialists (ADCES). Currently, we are able to enroll anyone with type 1 or type 2 diabetes and Medicare Part B. During this 6-week, virtual program, participants will learn ways to manage diabetes through goal setting, problem solving, physical activity, medication management, nutrition and more! A physician's referral is required; providers can complete the form online by clicking the arrow.
